Minors without a guardian: If you are under 18 and will not be accompanied by a parent or legal guardian (this includes relatives, grandparents, aunts, uncles, or adult siblings), you will need a notarized written authorization.
Late arrivals & insurance issues: If you are late or delayed due to referral or insurance issues, you may be asked to reschedule or placed in the next available rotation.
Appointment order: Patients are called back based on their scheduled appointment time, not their time of arrival.
Wait times: Although it is our goal to run on time, it is not always possible. If you can no longer wait, please stop at the front desk so we may reschedule you. We are truly sorry for any inconvenience. Please take a moment to read the article "Waiting for Good Docs" by F. William Danby, MD.
Non-patient visitors: For medical, legal, and ethical reasons, we cannot provide medical advice to anyone who is not the scheduled patient. Please stop at the front desk to schedule your own appointment.
School / work releases: If you require a school or work release, please notify staff before leaving the exam room. Releases will not be altered once issued; time off is determined by the doctor at the time of the visit.
90-day medication supply: If you require a 90-day supply for mail-in pharmacy, please notify staff at each visit. Some medications may not be available in a 90-day supply per doctor's orders.
Prescriptions: Dr. Agarwal-Antal will prescribe the medication she deems most appropriate. If cost or insurance requirements are a concern, please make this known at each visit.
Medical records: To request a copy of your records, a completed and signed HIPAA form is required. Per HIPAA guidelines, our office has up to 30 days to respond. Per Ohio State Law, fees are $3.51/page for pages 1–10, $0.75/page for pages 11–50, and $0.29/page for 51 and above (subject to change).
Refills: If a refill is requested outside of your scheduled appointment or on an emergency basis, a $25 charge per prescription applies.
Minor consent: If you are a parent accompanying a minor who may visit without you in the future, please ask staff about signing a Minor Consent Form so we may treat the minor in your absence.
Turning 18: If you are a patient soon to turn 18 and your parents will remain involved in your care, please notify the front desk so we can complete the appropriate authorization forms.
